The Financial Services Commission of Ontario (FSCO) has revised and updated the OAF 1 – Ontario Application for Automobile Insurance (Owner's Form) to reflect changes made to the data reporting requirements of the Automobile Statistical Plan (ASP).
Revisions
The data field specifying Automobile Use in Section 3 – Described Automobiles of the OAF 1 has been revised. The column for Business Use is changed to Business Use Percentage and the usual percentage of annual driving distance that is for business use is to be provided.
The changes made to the OAF 1 will make the collection of the data element, Business Use Percentage, easier and more specific.

Implementation
The revised OAF 1 will be effective for use beginning January 1, 2014.
How to Obtain the Revised Form
For your reference we are attaching a copy of the revised OAF 1 in both English and French. You can also access the forms at FSCO’s website at www.fsco.gov.on.ca.
