Insurance Forms

These forms are used by insurance agents, or by people who want to apply to become agents. The forms also enable agents to inform FSCO if they intend to make a change to the companies they represent, or if there is a change of address.

  • Agent Applications (New & Renewal) - Online version of harmonized application forms. To be used by insurers and agents to submit new or renewal applications for Ontario residents and non-residents applying for General, Life, and Accident & Sickness insurance agent licences in Ontario.

  • Licensing Link Agreement - Required to be submitted by insurance companies to enable Licensing Link access.

  • Insurance Company Prepayment Agreement & Form - Required to set up and operate a client services prepayment account for the pre-payment of insurance agent licence fees related to applications, renewals and transfers of insurance agent licences.

  • Insurance Adjuster Application - To be used by individuals to submit new or renewal applications for Ontario residents and non-residents applying for an Insurance Adjuster licence in Ontario.
  • Insurance Agent Corporate/Partnership Application - To be used to obtain a new/renewal life, general or A&S Corporate/Partnership licence in Ontario, it also enables a life, general or A&S Corporate/Partnership to inform FSCO of a change of name, address and/or sponsor.

  • Insurance Adjuster Corporate/Partnership Application - Only to be used to obtain a new/renewal Insurance Adjuster Corporate/Partenrship licence in Ontario.

  • Out of Province Agent Applications - Paper versions of the harmonized application forms, intended to be used by Ontario licensed agents applying to be licensed in another province, except for the Yukon, North West Territories and Nunavut, for General, Life, and Accident & Sickness insurance agent licences.
