Licensing Link has been updated to incorporate online versions of the harmonized application forms. To be used by insurers and agents to submit new or renewal applications for Ontario residents and non-residents applying for General, Life, and Accident & Sickness insurance agent licences in Ontario.
The paper versions of the harmonized application forms are intended to be used by Ontario licensed agents applying to be licensed in another province, except for the Yukon, Northwest Territories and Nunavut. Click on the required application below to print a copy from the CCIR website.
