This Bulletin highlights a number of changes and updates to the Settlement Disclosure Notice (SDN) form approved by the Superintendent for use in connection with accident benefit claims. This form has been amended primarily to reflect changes to the automobile insurance system that are effective June 1, 2016.

 

Amendments to this form are based on filed regulation amendments to the Statutory Accident Benefits Schedule - Effective September 1, 2010 and Automobile Insurance (R.R.O. 1990, Reg.664) and have been made in consultation with various industry representatives.
 
 

Highlights of Changes to the Settlement Disclosure Notice

 
Changes to this form include, but are not limited to, the following:
 
  • The descriptions of the benefits have been updated.
  • References to the Ministry of the Attorney General’s Licence Appeal Tribunal (LAT) have been added to align with the change in the dispute resolution process. 
 

Effective Date of the Form

 
The revised form is to be used on or after April 1, 2016.
