What changes can I make online to a Service Provider licence?

  • Changing the licensed business’ legal or operating name;
  • Changing the licensee’s Principal Representative;
  • Changing the licensee’s business ownership type;
  • Adding or removing a Director, Officer or Partner from the licence, as applicable;
  • Adding or removing a facility, branch office or location from the licence, as applicable;
  • Changing the address of a principal place of business or any facility, branch office or location on the licence, as applicable;
  • Surrendering the service provider’s licence.

What do I need to do to make changes?

To make any of the above changes to your Service Provider licence, you will have to submit a request to FSCO by logging into your existing FSCO account and completing an online application known as an update application.

Who can request a change to a Service Provider’s licence?

Any changes to the licence can be requested and authorized only by the service provider’s Principal Representative. The PR must complete and submit the corresponding supplemental application, as applicable.

How do I access a supplemental application to submit a change request to FSCO?

All supplemental applications can be accessed online through the FSCO portal by following the steps below:
  • Log in to your FSCO account and select the “Request a Change” link;
  • Select the respective application under “What do you wish to do” list to begin the process;
  • Complete the application and attestation, and submit the request to FSCO.
Depending on the change you are requesting, you may be asked to upload one or more supporting documents during the application process.

What happens after I submit my change request?

Once the request is received, FSCO will review the pertinent application on a first-come-first-serve basis. During the review, FSCO may contact you for clarification or additional documentation.

 

When the review is complete, you will receive an electronic notification outlining FSCO’s decision and/or outcome, as applicable. The notification will be available in the Message Centre of your online FSCO account.

Do I have to notify FSCO if I add or remove a regulated or non-regulated health professional from my business?

No. However, when you file your Annual Information Return you must disclose the number of regulated and non-regulated health professionals at the time of the filing.

How do I change my business ownership type?

You may only change the business ownership type from a Sole Proprietor to a Corporation online. The business’ Principal Representative must submit a request to FSCO by completing the online application titled, “Change Business Ownership Type.” During the application process, you must upload a scanned electronic copy of the Articles of Incorporation/Articles of Amendment or Articles of Amalgamation so it can be reviewed by FSCO.
